ordering your repeat prescription

In order for us to safely prescribe your medication and in line with NHS guidelines for issuing of controlled drugs we need to monitor you closely.  This means that we will require readings of your blood pressure, pulse and weight at regular intervals; this will be when you are initially seen in clinic, one week after you commence medication and on each repeat medication request (for at least 6 months) or until your GP agrees to a Shared Care Agreement.*

Your readings can be taken at home (you will need to provide photographic evidence), at your GP practice by your GP or Practice Nurse, or at a local pharmacy.

It is important that you email us your readings before we can issue your repeat prescription.

You are in control of ordering your own medication so to ensure you do not run short of medication we ask that you contact us with your request at least TEN DAYS before you are due to run out.

When ordering your medication you will need to provide the following information:

• Your Name
• Date of birth
• Home address (or address you wish medication to be sent to)
• Medication name
• Medication dose
• Any side effects or concerns
• Evidence of BP, pulse and weight measurements stating where and when the readings were taken (ie, name of practice/pharmacy and name of person taking readings). If measurements are taken at home we require photographs.
